COVID-19 Preparedness, Response, Health and Safety Plan

(Updated/Effective 6/24/2021) THE FOLLOWING WRITTEN PLAN TO ENABLE SOCIAL DISTANCING WITHIN THE OFFICES OF BOWERMAN, FORD, CLULO & LUYT, P.C. IS MADE IN COMPLIANCE WITH CURRENT GUIDANCE FOR COVID-19 FROM THE US CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) AND RECOMMENDATIONS IN “GUIDANCE ON PREPARING WORKPLACES FOR COVID-19,” DEVELOPED BY THE OCCUPATIONAL HEALTH AND SAFETY ADMINISTRATION (OSHA).
1. An employee shall self-monitor and immediately notify a member of the COVID-19 team if any of the following apply:
a) Whether the worker has COVID-19 symptoms including, but not limited to: fever, cough (excluding chronic cough due to known medical reason), shortness of breath, sore throat, and/or diarrhea.
b) Whether the worker had any close contact in the last 14 days with someone with a diagnosis of COVID-19. “Close contact” means someone previously designated as a “close contact” by the local health department during a case investigation, a household member, or someone within 6 feet for 15 minutes or longer in a 24 hour period starting 2 days before illness onset (or, for asymptomatic patients, 2 days prior to specimen collection) until the time the person is isolated.
c) Whether the worker has engaged in any international travel or has been in close contact with someone coming from outside the United States within the last 14 days.
d) Whether the worker has been directed or told by the local health department or their healthcare provider to self-isolate or self-quarantine.
e) An employee shall promptly report any signs and symptoms of COVID-19 to a member of the office COVID-19 team that develop during the employee’s work day.
